Patient Signalment

The patient signalment refers to the patient’s details that allows an accurate identification. This includes the patient’s name, species, breed, colour, sex, sexual status and weight (including body condition score). You should change the name of the patient and owner for confidentially reasons, or leave it out entirely.

An ideal example:

Name: ‘Molly’ Smith
Species: Canine
Breed: Border Collie
Colour: Black & White

Sex: Female, desexed
Weight: 15.2kgs
BCS: 3/5
